Sound Justice Initiative has partnered with the HEALI Social Care Network to help Medicaid members across Long Island access services through the NY Medicaid 1115 Waiver Program.

 

The NY Medicaid 1115 Waiver Program is designed to support individuals and families with health-related social needs. If eligible, members can be connected to resources such as nutrition support, housing assistance, and transportation services.

 

Below is a self-screening that takes a few minutes to complete. Your answers are confidential and will not affect your current benefits. If you can, have your Medicaid card ready before you begin. After you have submitted the form, the Sound Justice Initiative staff will review your responses and reach out to you via phone.
