Participation Requirements. Participants must provide updated contact information when such information changes and indicate the best times to contact them and whether messages can be left. Offer contact information of up to three people one of which, if applicable, must be a parole or probation officer who can be contacted for assistance with locating participant and give SJI consent to contact these individuals to obtained forwarding address, new email or phone number of the participant. Authorization to Reproduce Physical Likeness. Participant understands that photographs videotapes, live broadcasts, and other electronic recordings may be made during the program, which may include Participant and Participant’s likeness, voice, and/or sounds (collectively “Recordings”). Participant expressly consents to those Recordings and the use, transmission, public display, and public performance thereof. I understand that Sound Justice Initiative is the sole and exclusive owner of all rights, title and interest in the Recordings including all copyrights and other intellectual property rights. Privacy. Participant expressly consents to the collection and processing of personally identifiable data. Sound Justice Initiative promises that it will not rent or sell the information provided. However, I hereby grant to Sound Justice Initiative the right to use the personal information provided here for the purposes for which you provided the information and for general business purposes, including but not limited to program evaluation and/or other outreach activities, or to comply with legal processes. Sound Justice Initiative may also share non-identifying information provided with other organizations for purposes of strengthening its mission.
